Enrollment form For Happy Hound Pet Services Classes   Please complete the information below to enroll in our training program. To guarantee your space, please submit application and payment two weeks prior to class start date. Thank you very much for enrolling. Please click Continue button below to begin your enrollment form. Terms: As the legal owner of the dog you are enrolling, you are responsible for his actions during class. By completing this enrollment form you hereby waive and release Happy Hound Pet Services, Dawn Gardner, and Camp Bow Wow, and any affiliates and locations from any and all liability of any accident or injury resulting from the actions of you, your dog, or any guest that you bring with you to class.  I am the legal owner of this dog, and have read and accept the above terms.
